Common questions

What must a UK tattoo consent form cover?

We found no national rule that sets out what it must contain, so your council's byelaws or licence conditions come first. Beyond that, most artists record the client's full name and date of birth, whether ID was checked, the health questions that affect whether it is safe to tattoo today, the design, placement and date, that the aftercare advice was given, and a signature and date. Ask only what you will use: every extra question is more health data to look after.

Why is the age check on the form?

Tattooing anyone under 18 is an offence under the Tattooing of Minors Act 1969, with no exception for parental consent. The Act gives a defence if you had reasonable cause to believe the client was 18 or over, and a line on the form, with a note that you checked ID, is how you show that. Check the age against the day of the tattoo, not the day the form is signed.

How long should I keep consent forms?

UK GDPR sets no fixed period, and the ICO says you should not keep data indefinitely just in case. Health answers are special category data, so you need a lawful basis and a condition for holding them. If your insurer or council sets a period, follow it. Otherwise pick one you can explain, write it down, and delete forms once it has passed.

The law behind each line is in our guide to tattoo consent forms.
