The painted board outside the chandler's on Gull Steps had been there since the spring fair, its ribbon lettering flaking blue in the sea wind. Nobody in Porthkelly could say who had nailed it up, only that the man who answered to it kept a cart behind the net lofts and a slim rod of pale metal in his coat that hummed when he held it near a sick bed.

Mrs Adair brought her grandson down the steps on a wet Tuesday, the boy's chest rattling like a loose shutter. She had no coin to spare, so she offered eggs and a length of good rope. The tinker waved both away. He set the green-tipped rod against the boy's ribs and it sang a thin note, rising and falling like a gull over the bay, and the rattling stopped between one breath and the next.

Mrs Adair wanted to know what he was. He only tapped the brass collar and said he was a doctor of sorts, and that his people had always been particular about punctuality. Then he bowed to the empty doorway as if a crowd waited behind it, and told her the surgery would be closed until the spring fair.

The boy slept until noon. When the tide turned, Mrs Adair walked down to the lofts and found the cart gone and the board turned face down on the slipway. Someone had left a single ribbon of faded blue tucked beneath it. She kept it in her apron pocket for the rest of her days, and whenever a cough started in the village after that, she would stand in the doorway and say the words softly into the room, as though someone inside had been waiting for them.
