Fine line arrow tattoo on a forearm

Fine line · Forearm · Black and grey · Photo

A small arrow pointing upwards is drawn in thin black linework on the inner forearm, with a short crossbar near the head and feathered fletching at the tail. The arm is raised towards the head, with the hand in long brown hair, and the face is out of frame. The background is a soft-focus green hillside and pale beach, with the tattoo sharp in the centre of the frame.

The lift on Ward Six has been stuck between the second and third floors since Tuesday, so at ten past three I take the stairs with a trolley of clean linen and a sandwich I haven't unwrapped yet. Nobody on nights bothers with the arrows any more. The laminated ones taped to the walls were printed for a version of this building that no longer exists, and they point at a radiology department that moved to the old annexe in March.

Mrs Okonedo's grandson found me at the nurses' station, phone held out in front of him like evidence. He wanted the chapel. I told him I knew the way, which was true, and then I walked him the long way round, past the vending machine that hums in B flat, because he needed five minutes of not being frightened. At the fire door I took a biro from my top pocket and drew a small arrow on the back of his hand, tail feathers and all, pointing up the stairwell. He laughed, the first sound he'd made all night.

Upstairs, a porter has to think about direction differently. You stop looking at where you are and start looking at where the light falls. The east windows go grey before anything else does, and that is the way out, whatever the signs say.

At seven, when the day staff came on, my phone buzzed in the locker room. A photograph of his hand, the biro already smudged into a grey bruise, and under it a single line: found the chapel, and then the coffee. I folded my coat over my arm and went home the long way, up the hill, following the light.
