The monitor above bed nine has been drawing the same stubborn spike since midnight, and you have stopped pretending to read it the way a nurse should. Instead you hear it: a rise, a dip, a hesitation, the kind of gap a cellist leaves before the entry. Down the corridor the lift doors sigh open and shut again, and beyond the window Manchester glitters like a page of sharps and flats nobody has bothered to play.

Mr Abernethy, who has been awake for three nights and claims he is frightened of nothing except the ceiling, asks what you are humming. You tell him it is his heartbeat, more or less. He gives a dry cough of a laugh and says his late wife wrote her shopping lists on the back of hymn sheets, a treble clef at the top and a bass clef below, and insisted the two were the same heart in different keys.

You take a pencil from your tunic pocket and sketch on the back of the observation chart: a treble clef curling where the spike rises, a bass clef tucked against the dip, two dots beside it for the notes still to come. He says the dots are for the grandchildren. You tell him the line never really stops, it only changes key.

At half past five the trace settles into a slow, even walk. You fold the paper into his top pocket, go to fetch the tea trolley, and behind you the monitor keeps time in a key you have finally learned to hear.
