Anatomical heart with heartbeat line on back
Dotwork · Back · Black and grey · Photo
A stippled, dotwork anatomical heart with fine outlined vessels sits on the upper back, with a single black heartbeat line extending from its right side. The shading is built from dense dots in the heart and the trace is a crisp, thin line. The palette is entirely black and grey against pale skin, with the subject placed centrally and the shoulder and hair cropped at the edges.

The chart paper in bay four was running low, so Nadia Okafor tore off the last metre and held it up to the strip light. Someone had drawn a heart on the back in biro, a lopsided valentine from a child who had been on the ward in March. Beneath it, in a different hand, was Mr Pell's name and the words 'still ticking'. She pinned the sheet to the board above the trolley and went to fetch more.

Nobody on nights read the traces properly any more. The monitors did it for them, beeping their green verdicts, and the old cardiologists had retired to allotments and sea views. Nadia still liked the paper. She liked how each spike arrived on time, a small brave flick of ink, and how the flat stretches between them looked like breath being held. Her grandfather had worked a telegraph key in Lagos for forty years and used to say a steady line was just a rhythm somebody had agreed to keep.
At four, Mr Pell's trace went strange, the peaks bunching together like people at a bus stop. She rang through, the registrar came down in slippers, and for twenty minutes the bay was all quick hands and level voices. Then the line settled, rounder and slower, and the registrar sat on the edge of the bed to write his note.
Morning came in grey over the car park. Nadia rolled the spent paper into a tube for the bin and kept the valentine. The heart was only a heart, she told herself, and the message was only biro. But she folded it into her locker anyway, beside the spare batteries, so she would find it again at the start of every shift.
