Mind the ice on the third bend, the old woman said, though nobody had asked. Sit nearer the stove and I will tell you about Doctor Anselm Ruhl, who climbed the Varrin pass in the hard winter with a black tube looped round his neck. Every child in Scarrow swore the tube bent itself into a heart at the top, though the doctor said it only sagged that way from being carried too long.

The village had not seen a doctor in nine years. Mothers wrapped their babies in goat wool and listened to their chests with their own bare ears, which is no way to hear anything worth knowing. So when the doctor reached the first house in the snow, old Hesketh Brand pressed the cold round disc against his own grandmother's ribs and made her cough, and the room went quiet as a church.

He listened all that week, to the coughing and the breech births and the old men who only wanted someone to sit with them. Each night he wound the tubing back into its heart and tucked it in his coat. On the seventh night the pass closed behind him with a crack like a dropped rifle, and nobody could go up or down until spring.

So the village kept him. They taught him to bake bread that rose in the cold, and he taught them to count a pulse against the church clock. When the thaw finally opened the road, Hesketh's grandmother walked him to the top of the pass and gave him a flask of plum spirit. He put his ear to her chest one last time, listened a long moment, and told her the heart was doing its work. Then he went down the far side with the black tube swinging at his collar, and the villagers said the snow filled his tracks before he had gone a hundred steps.
